Thank you for your interest in re:new! The first step in being matched with a mentor is to complete a re:new packet, which includes a covenant, questionnaire, depression/anxiety test, and stress test. Once the packet is returned, we will schedule a time for you to meet with a staff person to discuss your situation in more detail in a confidential, safe environment. Packets are available by request at the Care Ministry table located outside the auditorium after any Sunday service, or you may print and complete each of the following:

1. Covenant
2. Questionnaire
3. AMEN Depression and Anxiety Test
4. Stress Test
